I have read the precautions that First Baptist Church of Mount Orab will be putting in place to protect my health and that of my family. I understand that I have an obligation to stay home if I or one of my family members are sick or have been exposed to COVID-19. Further, I understand that my participation is voluntary and that in spite of the precautions put in place, participation includes the risk of exposure to and illness from infectious disease including but not limited to influenza and COVID-19. I hereby RELEASE AND HOLD HARMLESS First Baptist Church of Mount Orab, its officials, officers, agents, and/or employees and other participants (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
